Strack enables intensive monitoring of patients with multiple pathologies during the chronic poststroke phase by utilising remote monitoring and networking between hospitals and primary care, employing a range of ICT tools and devices.
This comprehensive approach allows for better control of clinical, analytical, and biometric parameters, leading to reduced readmissions and visits, improved treatment adherence, enhanced physical and cognitive rehabilitation, and facilitating decision-making and coordination among care providers. Solution
At the time of discharge, nurses explain the Personalised Care Plan to the stroke patient, with access to an app, multimedia content and technological devices, according to severity, age and recovery capacity.
Remote monitoring allows a team of multidisciplinary professionals from the hospital and Primary Care to analyse the data and prescribe new content for the patient: physical activity guidelines, rehabilitation and cognitive stimulation exercises, etc.

The project in numbers
Our App
The app is an integral part of the digital solution for comprehensive patient management within the Strack project.
It facilitates patient monitoring by synchronizing several technological solutions established in the Personalised Care Plan by the medical and nursing team. These include digital scales for weight measurement, a heart rate sensor for monitoring HTA (hypertension), a Holter for detecting arrhythmias, and Google Fit for tracking physical activity, among other clinical, analytical, and biomedical parameters.
Additionally, the app provides a library of health content prescribed by the management team based on the personalised Care Plan.
