Modify a patient's contact details

The patient's affiliation and contact details are very important to facilitate contact between the patient and the Hospital Universitari de Bellvitge. For this reason, we must be rigorous in modifying them.

Fill in the following form to update the contact details (change of address, telephone number, etc.) that appear in the clinical history of a patient at the University Hospital of Bellvitge.

We recommend that you also communicate this change to your primary care centre (CatSalut). If the data in the clinical history of the Hospital Universitari de Bellvitge do not coincide with those in the CatSalut register, the data in the CatSalut register can be used as a reference. You can make this change at any CatSalut centre.
